I, representing my company, (hereinafter referred to as "Subcontractor"), hereby declare that the information provided in this form is true and accurate to the best of my knowledge. I understand that by submitting this information, I am agreeing to comply with all applicable rules, regulations, and terms outlined in the subcontractor agreement with Imperial Road Haulage Limited.

I acknowledge and understand that failure to adhere to the specified rules, regulations, safety standards, and contractual obligations may result in the termination of the subcontractor agreement between Imperial Road Haulage Limited and my company. I also understand that I am responsible for ensuring that my vehicles follow all relevant laws and regulations during the course of the contract.
